- Bring your last dialysis prescription.
- Arrive early to complete onboarding.

What should I bring to my visit?
Bring ID, any medical/referral documents, and a list of medications. For dialysis, your care team will guide you on specifics.
Do you offer kidney screening?
Yes. Screening is recommended for persons with diabetes, hypertension, or a family history of kidney disease.
